D/T Submission Checklist

In order to meet Graduate School requirements for formal submission of a dissertation or thesis, your document and all accompanying materials must be submitted by 5 PM on the deadline date. If an adviser cannot provide an original signature by the due date, 
or if there will be any other problems meeting the deadline, your adviser must email Shari Hill Sweet, with an extension request before the deadline.

This checklist assumes your manuscript’s formatting has been approved by the formatting editor. If you haven’t had the formatting checked, please submit a PDF or paper copy to the editor before proceeding. If the formatting of your formal submission does not match the requirements set by UMI/ProQuest and the Graduate School, your dissertation or thesis may not be accepted.

All checklist items are required unless otherwise specified. Checklist items can be completed in any order; we will not, however, accept a dissertation or signed title pages submitted prior to the defense.

Submission Components
Doctoral Dissertations Master’s Theses
  • The dissertation: Upload 1 PDF to the ETD website
  • Title pages: 2 printed title pages, both with the research director’s original signature and the month and year of formal submission at the bottom of the page
  • Submission payment for a doctoral dissertation. This payment covers the both Graduate School and ProQuest fees.
  • Graduate Career Outcomes Form
  • ProQuest Microfilming Agreement (pages 4 and 5; page 6 if you’re ordering copyright)
  • Exit Survey and Survey of Earned Doctorates: Survey invitations are sent out with the initial graduation list midway through the semester. Watch your Notre Dame email address for updates.
  • The thesis: Upload 1 PDF to the ETD website
  • Title pages: 2 printed title pages, both with the research director’s original signature and the month and year of formal submission at the bottom of the page
  • Submission payment for a master’s thesis. This payment covers both Graduate School and ProQuest fees (if applicable)
  • Graduate Career Outcomes Form
  • ProQuest Microfilming Agreement (optional — but if you order it, we need pages 4 and 5, and page 6 if you’re ordering copyright)



The approval, cataloging and microfilming processes take time, so it may be several weeks before your dissertation becomes available in the University library system or on the UMI/ProQuest website. If there are any problems with your submission, the dissertation editor will contact you to resolve the issue.
