In an effort to make the affiliation process easier with all of the issues involved with the new system, please use this writable affiliation form below and email it to affiliation@fcclainc.org or mail it to: FCCLA, Lock Box Operations, P.O.Box 718, Dept.D, Baltimore, MD 21203-0718. Affiliation Form 2011-2012 Chapters that have already affiliated using the online system can easily access a copy of their invoice. Login to the affiliations system and on the top left click on "Print Invoices", your invoice is available to print.
