HSHS Medical Group, launched in 2008, is a growing network of primary care groups, multi-specialty groups, single specialty groups and independent physician and midlevel providers in Illinois and Wisconsin, working alongside Hospital Sisters Health System (HSHS) to create a new, integrated approach to health care delivery.
As a critical component in HSHS’ Care Integration strategy, HSHS Medical Group seeks to create a culture of shared responsibility, partnership and co-management among our medical professionals. Strategic goals include:
- Define an integrated patient care model that meets the needs of those we serve, as well as those who provide care.
- Collaborate with existing HSHS physician and midlevel partners to expand our network of partnerships to additional physicians, midlevels, nurses and clinicians who share our vision of integrated health care.
- Develop new strategies to improve efficiency and reduce waste.
HSHS Medical Group was designed by providers to meet the needs of providers, and ultimately their patients. We are committed to a provider-led, and provider-governed organizational structure in which providers practice with “disciplined autonomy.” This simply means that it is your practice and the medical group creates structure to help you succeed.
We are committed to ensuring that providers are linked into a seamless network of providers in a variety of practices in our community. In addition, providers are given the latest tools and technology needed to deliver excellent care—as well as a professional and nurturing work environment.
