Diabetes affects nearly 24 million Americans and almost 300 million people around the world. Each year, about 1.6 million people in the U.S. aged 20 or older are newly diagnosed with the condition. Early detection and consistent management are critical in controlling the condition and preventing it from leading to other serious health complications.

Care4life^SM is a personalized interactive mobile health service designed to help diabetics manage their condition by leveraging the ubiquity of mobile phones to educate, interact, and engage the patient. Developed with content from the National Diabetes Education Program, a partnership of the National Institutes of Health and the Centers for Disease Control and Prevention, Care4Life leverages Voxiva’s robust multi-channel communication platform to engage and interact with the patient.

Enrollment:
Participants can self-register for this service via mobile phone or web, or they can be enrolled by a third party sponsor such as an employer or health plan.

Individualized Profile
Once enrolled, participants completes a health risk assessment which is used to place them in an appropriate educational protocol. No two patients are the same, and these assessments help to customize the patient’s management plan and personalize their experience.

Targeted Messages
Care4Life delivers personalized and targeted messages to encourage adherence and educates the user about their condition. It also provides links to resources that a participant can access to support their condition.

Targeted Reminders
Care4Life is designed to improve adherence to treatment regimen, and participants are prompted at regular intervals to check and record their glucose levels. Users can also enroll to receive reminders to take their medications as well as schedule appointment reminders alerts.

Diabetes Diary
Care4Life allows participants to record their test results via text message directly through their mobile phone or through their personal web portal. By responding to daily text alerts, participants can build up a record of their blood glucose over time. The inputs in the Diabetes Diary are used to deliver targeted and customized content. The records can also be shared with a care provider.

Personal Health Portal
Care4Life analyzes the Diabetes Diary and provides personalized charts to map the results against a pre-defined goal. Patients can track their progress overtime and adjust their treatment regimen accordingly.

Device Integration
Voxiva’s Care4Life can be integrated with wireless devices to help the user conveniently upload their test results in real-time directly from their glucometer and into their Diabetes Diary. This augments the user experience and helps the patient to monitor their condition more effectively.
