    A job hazards questionnaire (JHQ) was generated on your behalf prior to your appointment with the division. Required environmental safety and health training courses are driven by answers provided on the JHQ. An ANL domain user name and password is required to login, view and make changes your JHQ. Answers provided on your JHQ should reflect current duties and responsibilities in order to avoid unwarranted training requirements.

    Training courses specifc to the HEP division may require your login and include the following:

    • HEP/Bldg. 360
    • HEP/Bldg. 362
    • HEP/Bldg. 366
    • HEP101/AWA Accelerator Operator OJT
    • HEP102/AWA Laser Operator OJT

    All training requirements are managed and maintained within the Training Management System (TMS) a Windows-based computer application operated by the ESQ division and designed to identify training needs/courses based on workplace hazards and responsibilities.

    If you plan to do work, conduct an experiment, build, construct and assemble equipment, operate machinery or perform a non-experimental task, all the above require Integrated Safety Management(ISM)and Work Planning and Controls(WP&C) as established in conjunction with the Laboratory Management System(LMS).

    Incident Reporting: When an incident occurs take one of the following action(s):

      A) If the incident is an emergency;
        1. On-site from an Argonne land line, call 911.
        2. On-site from a cell phone, call 630-252-1911.
        3. Off-site, call the location-specific 911.
      B) If the incident involves injury and not an emergency, follow the steps listed below:
        1. Use a bandage for self-treatment if appropriate.
        2. If computer access is readily available, complete and submit the incident notification web form within one hour of stabilizing the injury/illness, regardless of the day or time.
        3. If computer access is not readily available, call the incident reporting hotline at 811 from any Argonne phone or 630-252-1811 from a cell phone or outside line to report the incident within one hour of stabilizing the injury/illness, regardless of the day or time. The hotline is staffed from 7:00 a.m. to 7:00 p.m., Monday-Friday; after hours, follow the voice mail instructions.
