Get a U.S. Passport
The Passport Application Acceptance Service is offered by appointment or on a first-come, first-served basis at the Main Library at 251 South 'A' Street during the hours below.
Basic Requirements
(Additional documentation may be required)
- Completed passport application: Do not sign. DS-11 form (complete items 1-22 only) You may download the form at travel.state.gov.
- One (1) passport color photo (2 in. x 2 in.). Passport photo service available during Passport Acceptance Office hours ($10 cash per person).
- Proof of citizenship: Birth certificate (must be certified), previous U.S. Passport, or Naturalization Certificate (full size only, no abstracts or copies will be accepted)
- Current I.D. with photo
- Two (2) separate payments: Check or money order to Department of State and cash to the City of Oxnard for the execution fee.
NOTE: All applicants must be present.

This office does not accept passport renewal forms.
FEES*
A Passport Book is valid for travel by air, sea, or land.
A Passport Card is valid for travel by sea, or land between the United States and Canada, Mexico, Bermuda and the Caribbean only. It is not valid for air travel.
Fees for Passport Books and Passport Cards must be paid in two (2) payments as follows:
- The application fee must be in the form of a check or money order made payable to DEPARTMENT OF STATE.
- The execution fee shall be paid to the City of Oxnard in CASH.

NOTE: Both parents must be present when applying for a passport for a child under 16 years of age.
