Fremont Family Care
Fremont Family Care, a small primary care practice in Fremont, NE is a 2014 HIMSS Community Health Organization Davies Award recipient. Using the EHR, the practice began tracking patients due for preventive care such as mammograms, colonoscopy and an annual physical. The rate of patients’ age 50-75 that had a colonoscopy rose from 37 percent in the first quarter of 2011 to 70 percent in the second quarter of 2014.

Located in White River Junction, Vt., the practice has a team of six family physicians, three family nurse practitioners and an administrative support staff of fourteen.

Unity Health Care was named a 2012 HIMSS Davies Community Health Award recipient. As the largest CHC network in the District of Columbia, it serves nearly 95,000 underserved residents in 30 sites. According to the Davies information, Unity implemented a new phone system, piloted centralized scheduling, and implemented an EHR/PM system, contributing to a 21% increase in overall provider productivity.
