STEP 3: Financial Aid Enrollment/Application Review
- Submit IRS Form 1040
Parents’ most recent IRS Form 1040. This is used to confirm amount of financial aid applicant will receive.
STEP 4: Interview
If the Admissions Team and parents conclude that Paul Anderson Youth Home is a possibility for the applicant and his family, an interview will be scheduled. Following the interview, if it is determined that the needs of the young man are within the area of expertise of the PAYH, the young man will be given a date for admission into the program and will be asked to submit the items found in step 4.
A $300 interview and assessment fee is due on the day of the interview. This may be paid with either check or credit card.
Prior to intake the following documents must be submitted:
- MEDICAL RECORDS
A completed PAYH Medical Form that is less than 30 days old - IRS Form 1040
- COURT RECORDS
Court order as well as documentation of all charges and their disposition - CERTIFIED COPY OF BIRTH CERTIFICATE
- COPY OF DRIVER’S LICENSE OR STATE ISSUED ID
- SOCIAL SECURITY CARD
