Sample Submittal Form: Foam Baby and Children’s Products

Thank you for your interest in this project. This Citizen Science Corps survey will sample a small piece of foam from baby or children's products such as car seats, nursing pillows, bassinet mattresses, crib wedges, and changing pads to identify the presence of halogenated flame retardants. To send us your foam for testing, please fill out the form below or download a form and mail it in.

Instructions for submitting a sample:

Cut a 1 square inch piece of foam from your product.
Place it in a sealed plastic sandwich bag.
Mail it to us with a copy of this form.

Once you submit this form, you will receive an email with a confirmation number. Please include this number in or on the envelope. Once your package arrives, your samples will be analyzed using High Definition X-ray Fluorescence (XRF) and other methods if they are available. Foam samples will not be returned to you after testing and may be damaged as a result of testing.
