How to be Referred

Most referrals to Saint Francis Hospice come via GPs, hospitals and district nurses

I am a healthcare professional and would like to download a referrals form

We accept referrals by fax or by post from any healthcare professional such as your GP, hospital or district nurse.

We may need to speak to the referring professional for additional information prior to initial telephone contact to patient/family. 

Referrals can be made for any of our services both at the hospice and in the community which are all provided free of charge to patients and their families.  Every referral is assessed at our regular referrals meeting to assess which service has been requested and is most appropriate.

We make every effort to ensure that patients receive individual care that is right for them.  If appropriate we will liaise with your GP, district nurse or any other healthcare professional involved in your care.
