Gold STAMP Program: Pressure Ulcer Improvement

LeadingAge New York joined the Department of Health (DOH) in a statewide collaborative called the Success Through Assessment, Management, and Prevention (Gold STAMP) Program to provide evidenced-based resources and education to hospitals, nursing homes, and home- and community-based providers about a systems approach to pressure ulcer prevention, assessment, and management.

Pressure ulcers are a significant patient safety issue, are high-cost, and have a negative impact on quality of life for individuals across the continuum of care. Pressure ulcers have been a focus of quality improvement initiatives in nursing homes and home care for some time. Monitoring and reporting of hospital-acquired pressure ulcers have increased their profile as a patient safety issue for acute care.

Studies by the National Quality Forum and Agency for Healthcare Research and Quality indicate that hospital treatment of a pressure ulcer increases the average cost of a hospital stay by $1,200 per day, and about 54% of patients hospitalized in 2006 with a primary or secondary diagnosis of pressure ulcers were discharged to a nursing home. The research also shows coordination of interventions across settings plays an important role in pressure ulcer prevention and management.

Gold STAMP facilitates collaboration between all settings on a systems approach to improve pressure ulcers for patients, by providing resources, tools, and opportunities for providers to partner together.
