Palliative Care is a service for patients with serious or life threatening illnesses
aimed at relief of symptoms and improvement in the quality of life throughout
the course of their illness or condition. Conditions appropriate for the service
may include but are not limited to cancer, heart and lung disease.

Physician Orders for Life-Sustaining Treatment (POLST)
POLST is important for people with serious health conditions. It is used
to make a person’s wishes for medical care known to doctors, nurses, emergency
medical personnel and other healthcare staff. Your doctor uses the POLST form
to write orders that indicate the treatment you want in the last stages of an
illness.
